When you call our offices to set up your initial appointment, our receptionist will request your email address, phone number and date of birth. As soon as a chart note is recorded in your electronic medical record with us, you will receive an email invitation with instructions on how to register in the patient portal.
You can then register from your computer or a mobile device.
The email will provide you with instructions on how to create your account on Patient Fusion. After clicking the Create your account button, you will be able to choose if you want to enter the PIN from the doctor or request a mobile security code with your phone (via text or voice).
You will then either have to verify your identity using your date of birth (in MM/DD/YYYY format) and the PIN the doctor provided you or the phone number you gave our office.
After verifying your identity, you will have the choice to link your record to an existing Patient Fusion account or create a new account. Linking an existing account allows you to view records from multiple health providers of yours who might also be using the Patient Fusion platform and, if you are authorized, the records of other individuals, such as your children or spouse, with one account.
Finally, you will select and answer a security question, and put in a username and password. Your default username will be pre-populated as the email address you provided us. Passwords should be a minimum of 8 characters and should contain one uppercase letter and a number or symbol.
Please keep record of your password so you’ll have it handy when you need to log-in to your account.
Please keep in mind at this point you have the ability to alter your username. If you receive the error message "Username has already been taken," you will need to alter your username.
At this point, you will be logged in to your account, where you can access your health records. You will see any diagnoses, medications, immunizations, allergies, procedures or care plans your doctor has recorded as structured information, as well as any lab results you have explicitly shared with them and upcoming or past appointments.
